Coaches
All teams must hand in the one of attached completed checklists
(there are 3 different checklists in the file - use the one that applies to your
team) Please make sure that you sign the document. CUSL requires this checklist
in order to process your paperwork. No checklist and your team's paperwork will
NOT be approved by CUSL.

CUSL is now requiring that every coach fill out a new Ohio South RISK
MANAGEMENT Form On-line. The new requirement applies to all coaches. |
